Schedule appointment on Calendly; contact me ahead of time if you are unable to attend that visit
Get old records sent to me including any Special Issuance letters from the FAA or labs and letters from your primary care doctor for any medical issues for which you are being treated. The confirmation letter from Calendly should have a link for a release of information for OhioHealth if you are a previous patient of mine.
Complete the MedXpress at least 48 hours prior to your appointment for my review
Complete payment is mandatory prior to the exam being performed so bring a credit card for scanning to the appointment
